Hip joint stiffness is a musculoskeletal condition characterized by a reduced ability to move the hip through its full, normal range of motion. Rather than a single diagnosis, hip stiffness is best understood as a clinical presentation — a symptom that can arise from dozens of underlying causes. You may notice it as difficulty bending forward to tie your shoes, an inability to cross one leg over the other, or a deep ache in the groin after sitting for long periods. The sensation is often described as a "catching" or "tightness" that limits everyday movements such as walking, climbing stairs, or getting in and out of a car.
Clinically, hip stiffness is measured by comparing a patient's active and passive range of motion against established norms. Normal hip flexion is approximately 120 degrees, internal rotation about 35 degrees, and external rotation about 45 degrees (Radu et al., 2022, PMC9026847). When any of these arcs are restricted — whether by pain, capsular tightening, bony changes, or muscular guarding — the hip is considered stiff. The restriction may be symmetrical or isolated to a single plane of motion, and this pattern often provides valuable diagnostic clues about what is causing the problem.
Hip joint stiffness affects people across all age groups and activity levels. In younger, active populations it is frequently associated with femoroacetabular impingement (FAI) or labral pathology (Dancy et al., 2025, PMC11939100). In middle-aged and older adults, osteoarthritis is the leading contributor, with prevalence increasing sharply after age 55. Regardless of the cause, early physiotherapy intervention has been shown to slow the progression of stiffness, reduce pain, and preserve or restore functional mobility (French et al., 2022, PMC9574868).
How Hip Stiffness Differs from Hip Pain
Although stiffness and pain frequently coexist, they are distinct phenomena. Pain is a sensory experience driven by nociceptive or neuropathic signalling, whereas stiffness reflects a mechanical limitation — the joint simply cannot travel through its expected arc. A patient may have significant stiffness with only mild discomfort, or severe pain with near-normal range of motion. Distinguishing between the two is critical because the treatment priorities differ: pain-dominant presentations benefit from analgesic strategies and load management, while stiffness-dominant presentations call for targeted mobility work, manual therapy, and progressive stretching.
The hip is a ball-and-socket synovial joint formed by the spherical head of the femur (thighbone) and the concave acetabulum of the pelvis. This architectural design provides an exceptional combination of stability and mobility, allowing the leg to swing forward and backward, move out to the side and across the body, and rotate inward and outward.
